The input items on the booking page can be customized by adding them to the basic items.

How to set up
Go to Edit Booking Page > Basic Settings > Reception Form,
Basic Settings > Reception Form.
Selecting "Add item" allows you to set a question item.
You can also set the required fields.
Items can be selected from descriptive, radio buttons, check boxes, pull-downs, etc.
